Please read this Consent Form carefully before giving consent. The following information will be collected during the registration: Name, date of birth, gender, address, email, mobile number, medical history, medical reports, prescriptions and health information. Information given to us is stored on secure servers with policies and procedures to maintain the safety and security of that information.

By signing this Consent Form, I acknowledge and agree that:
  • I consent to completion of my HRA and/or to the collection and maintenance of my personal information by Health Meter Services Pvt Ltd, its affiliates and the empanelled hospital and diagnostic centers.
  • I understand the information collected may be used by Health Meter Services to generate a personalized user interface for my health analysis.
  • Health Meter Services may use the information in aggregate for the purpose of generating reports for your company or full population. These aggregate reports on the full population may be shared with other parties. Your personal information will NEVER be sold or given to any other party.
  • To the extent that I have questions concerning this statement, I have contacted Health Meter Services and those questions have been answered to my satisfaction.
  • I understand that myhealthmeter mobile application is an educational tool and NOT a substitute for a doctor or physical examination. It is my responsibility to communicate with my family doctor to review the reports and concerns.
